POSTION DESCRIPTION

Position Title: Charge Nurse : LPN   

Departmental Title:  Personal Care

Supervisorís Title:  Nursing Supervisor/Unit Manager  


POSITION OBJECTIVE:          

LPN Charge Nurses are responsible to organize, deliver and direct professional nursing care to residents.


REQUIREMENTS:

1. Graduation from an accredited School of Nursing as a Licensed Practical Nurse.
2. Current LPN licensure by the Pennsylvania State Board of Nursing.
3. CPR certification required (prior to or upon employment).

 

KEY RESPONSIBILITIES:

1.  Serves as charge nurse for assigned resident care unit assuring continuous clinical leadership at all times. Directs NAs regarding resident care and notifies families, physicians and departments of residentsí needs.

2.  Collects data on residents and accurately documents residentsí condition in an informative and descriptive manner that reflects the care provided to the resident, the residentís response to the care, and medical assessment of the resident. Completes assigned  paperwork, i.e., wound documentation, MARS, TARS, Care Plans, etc. on each resident on assigned resident care unit.

3.  Performs clinical testing such as glucometer and pulse oximetry as necessary. Administers ordered medications and treatments as approved, including monitoring of IV fluids.  Monitors effectiveness of medications and treatments.

4.  Prepares for and performs weekly resident rounds with the physician. Accepts physicianís orders including verbal/telephone orders as appropriate and compliant with regulations and nursing practice. Transcribe physicianís orders to residentsí charts and care plans as required.

5.  Assists in completion of 24 Hour Change of Condition Roster.

6.  Maintains CPR certification. Initiate CPR in compliance with Masonic Villagesí policy.

7.  Reports to on-coming shift and receives report from prior shift concerning the condition of  residents and any unusual occurrences. Is accountable for counting narcotics at change of shift with another licensed nurse and reporting any discrepancies.

8.  Participates in resident care conferences as needed.

9.  Participates in the orientation of new nursing department personnel.

10. Consults with Nursing Supervisor or Director of Nursing as appropriate regarding specific nursing policies, interpretation of policies or suggestions/concerns regarding residents or employees including any concerns of possible resident abuse.

11.  Participates in the maintenance of a clean and safe environment. Supports the infection control program.

12. Assumes responsibility for maintaining current knowledge and skills with respect to facility initiative, nursing procedures and technology.

13. Practices fiscal responsibility in the use of supplies and equipment and time management.

14. Attends in-service programs and staff meeting.

15. Assist with direct resident care including bathing, feeding, making beds, toileting, answering call lights and transporting residents to appointments and activities.

16. Performance post-mortem care as needed.
